Act 723 of 2007 - Phase I
Districts completed before 06.30.10

Due September 30, 2011, as part of Cycle 1, each district signed and submitted a Cycle 1 Certification of Data Accuracy form.  On that form it was indicated that the district had printed the Phase I sheet and placed in the Act 723 folder in the district central office.  Each district must keep this page on file with the district's current plan to use as a basis for modifications.  Follow the complete instructions found on the Act 723 link on the menu.  ADE no longer has copies of Act 723 Phase I sheets as each district Superintendent has signed that the list and plan has been placed in their local office for availability.
